Certified Food Protection Instructor Requirements

Due to irregularities and inconsistencies in reporting procedures, the Environmental Health Services Division of the Washoe County Health District (WCHD) standardized the process for instructors providing Certified Food Protection Manager (CFPM) information. WCHD now requires instructors to fill out the Food Protection Instructor’s Certification of Student Hours form for every class.

Instructor required form for classes or proctored exams conducted:

The form streamlines the process for instructors and their students, and ensures accuracy in reporting and enforcing regulations. If the form and attachment from the accredited agency (ServSafe, Prometric or National Registry of Food Safety Professionals) are not received by our office, WCHD will deny students a card and refer them to contact their instructor.

Please call 775-328-2645 or email us using the form below with any questions regarding the requirements for the CFPM Certificate process. Paperwork can be submitted via mail, fax or email.
