Daybreak (Adult Day Healthcare Program) provides group care during the day (M-F) in a safe, supportive, structured and therapeutic environment for adults with disabilities (18+) and provides a wide spectrum of services under the supervision of a qualified registered nurse. The program can provide the level of care that many family members seek as an alternative to more restrictive placements or institutional care. It is a secured site with the capability to handle clients with behavioral problems or wandering tendencies. At the present time, our program is the only adult medically based model in northern Nevada offering nursing services for health maintenance and chronic illness monitoring.

Daybreak can be summarized as follows:

  • Designed for adults who are frail and/or physically, cognitively or emotionally impaired with conditions such as heart disease, stroke, Parkinson's, diabetes, arthritis, traumatic brain injuries, multiple sclerosis, Alzheimer's and other dementias.
  • Emphasizes keeping participants at their highest degree of independence and well being
  • Brings isolated participants together in a group to promote socialization, provide peer support, strengthen self-esteem and prevent mental and physical deterioration
  • Provides assistance with personal care and other acitivies of daily living
  • Supports affordable care in a community-based setting as well as controls costs
  • Allows frail elderly clients to remain in their homes delaying institutional placement
  • Provides a supervised, protective, congregate location for families/caregivers who can not leave their loved one home alone during the day
  • Allows caregivers to maintain employment and have time to care for themselves or other family members
  • Provides structured programming throughout the day
  • Capacity for 33 clients

The group setting aids participants in overcoming the isolation, which often occurs with illness, and for achieving and maintaining physical and emotional health. Participants may attend one or more days a week, Monday through Friday from 7:30 AM to 5:30 PM.

Daybreak provides a lower cost alternative to nursing home care, which is not always medically necessary or psychologically satisfactory. Services provided include needs assessment, nursing services, rehabilitation services, psychosocial intervention, activity programs, and nutritional services. We are committed to enriching the lives of our participants by providing quality services in a safe, secure environment and preserving the dignity and independence of our participants. Our focus is to provide an invigorating community setting in which teamwork and creativity nurture all aspects of well-being for each individual program participant.

Prospective participants may apply directly to the Daybreak program or a physician or community agency may refer them. Participants must be 18 years of age or older and have some form of physical or social disability to the degree that they cannot stay alone during the day, but do not need 24 hour a day inpatient care. They must be under a physician's care and have had a recent medical assessment, and be able to walk or use a walker or wheelchair. Daybreak is unable to accommodate anyone who is bedridden or unmanageable to the point of being a danger to him/her self or others. Persons without a doctor are assisted in obtaining medical care. Entrance to the program is dependent on available space and completion of the medical assessment and required forms.

Daybreak receives funding through a variety of sources:

  • Washoe County
  • The State of Nevada
  • The federal government
  • Program Income
  • Donations

The regular program expenses are covered by these funds, but do not allow us to replace items that are worn out from age or use or allow us to make improvements to the facility or progamming. Below is a list of improvements that we look forward to doing once funds become available. If you wish to make a donation, please call 328-2591.

Program Needs --

  • Bathroom with shower to allow for thorough cleansing of incontinent client(s). Other bathrooms need to be painted after removal of vinyl wallpaper as it can no longer be thoroughly cleaned
  • Outdoor patio with shade and fencing on the east side of the Senior Center so clients can enjoy the outdoors in a safe and secure environment
  • Exercise equipment to help foster clients' strength and support (A Fall Prevention Program)
  • Curtains - current ones are 15+ years old and can no longer be cleaned
  • Storage cabinets for programming equipment storage
